The role of leukotriene modifiers in the treatment of asthma.

چکیده
Asthma is a complex syndrome characterized by variable obstruction to airflow, bronchial hyperresponsiveness, and inflammation. The initiation and propagation of airway inflammation arises from many factors, including mediators generated by resident airway cells and recruited leukocytes (1). Leukotrienes are biologically active fatty acids derived from the oxidative metabolism of arachidonic acid, an integral component of the cell membrane (2). A role for leukotrienes in the pathogenesis of asthma has been suggested by their biologic activities, which produce effects that mimic those of clinical asthma, and by the effects of either inhibition of leukotriene production (5-lipoxygenase inhibitors) or antagonism of leukotriene binding to cellular receptors (leukotriene D 4 -receptor antagonists). The recent U.S. Food and Drug Administration (FDA) approval of a leukotriene-receptor antagonist, zafirlukast (Accolate), and a leukotriene synthesis inhibitor, zileuton (Zyflo), provides the first mediator-specific therapy for asthma. This review will consider the biochemistry of the leukotrienes, their biologic role in asthma, and the therapeutic potential of drugs that alter the production or action of leukotrienes, as well as provide guidelines for the use of leukotriene modifiers in patients with asthma.

The Role of Leukotrienes in Respiratory Tract and Asthma
Polyunsaturated fatty acids play a role as precursors of biologically active compounds that can act as mediators or modulators of various cell functions. Thus three main groups of derivatives the prostaglandins, the thromboxanes, and recently discovered leukotrienes are formed by oxygenation and further transformation of various polyunsaturated fatty acids of which arachidonic acid plays t...
متن کاملThe Efficacy of Montelukast in Adjunct with Amoxicillin in Treatment of Acute Otitis Media (AOM) in Children
BackgroundAcute otitis media (AOM) is very common in children. In spite of spontaneously relief, some cases suffer from the recurrence and other complications occurring after AOM. Animal studies have proved that persistence of leukotrienes in the middle ear may play a role in the development of AOM. Based on this statement, treatment with leukotriene modifiers may be effective in the treatment ...
متن کاملSafety and efficacy of montelukast as adjunctive therapy for treatment of asthma in elderly patients
Asthma is a disease of all ages. This assumption has been challenged in the past, because of several cultural and scientific biases. A large body of evidence has accumulated in recent years to confirm that the prevalence of asthma in the most advanced ages is similar to that in younger ages. Asthma in the elderly may show similar functional and clinical characteristics to that occurring in youn...
متن کاملMontelukast: its role in the treatment of childhood asthma
The cysteinyl leukotrienes, LTC(4), LTD(4), and LTE(4), play an integral role in the pathophysiology of asthma. Acting via the type 1 leukotriene (CysLT(1)) receptor, these proinflammatory mediators have numerous effects in the lungs, including decreased activity of respiratory cilia, increased mucus secretion, increased venopermeability, and promotion of eosinophil migration into airway mucosa...
متن کاملDetermine Efficacy of a Short Course of Montelukast in Children with Intermittent Asthma and Viral Infection
Introduction Mild intermittent asthma is common in children and viral infections are responsible for the majority of exacerbations. As leukotrienes are potent inflammatory mediators, some studies have shown that Montelukast, a leukotriene receptor antagonist, may be effective on reduction of asthma symptom. To determine whether a short course of Montelukast in asthmatic children with common col...
